Join us for a highly participatory FREE six-week Diabetes Self Management Program, designed for anyone 60 years or older living with type 2 Diabetes or Pre-Diabetes, or has family members with Diabetes.
This program emphasizes the skills needed in day-to-day management of Diabetes, including strategies to increase or maintain activity levels. You will learn along with your peers, to prevent low blood sugar, nutrition and meal planning, talking to your doctor, weekly goal setting, and stress management.
This free six-week class will be on Mondays, 10AM-12:30PM, beginning on April 9, 2018.
